BARP, R foot knuckled. Hopped around kennel when I went to grab. Gave boots on each foot. NOLO. Changed papers.
VET NOTES Brought in to repeat radiographs, check foot, etc. The left foot is still red and there is a dark area on the medial aspect of the plantar surface. According to staff he rests his right foot at an angle on the perch and I suspect is not able to fully weight bear. Radiographs show some sharp edges still around the fracture site, with at least one fragment medially not incorporated into the callous. Overall, it doesn't look like there is good callous radiographically and it still feels slightly unstable palpably. Also, the bone does curve inwards somewhat. Although I couldn't make a side by side comparison, it also looks as though the cross-pins are bending somewhat as the fracture has collapsed a bit. Unable to measure the

Veterinary Recheck, Right Femur Fracture, Radiographs:
Palpation of right femur shows palpable callous formation and good stability with the external fixator and associated pins. The skin sutures are still in place and the skin incision is fully healed so the skin sutures were removed today.
Brief general anesthesia, mask induction and maintenance.
Recheck radiographs today show good alignment of the femur, there appears to be early callous formation. The butterfly fragment is still visible adjacent to fracture site and this has not yet been incorporated into the callous site.
Plan is to continue to house bird indoors due to the upcoming very cold weather.
Recheck in about 2 weeks (radiography) to evaluate further for healing.
All remaining oral meds can be discontinued after today.
Recovery from anesthesia was smooth and uneventful.

VET NOTES Bird is standing on perch, leaning to the left to offload weight from the right foot, but placing the right foot normally. Right wing is a bit drooped and gets caught up in the external fixator. Did PT, flexing and extending leg at hip and stifle. Good ROM and I thought the joints loosened up fairly readily. Cleaned pin sites - there may be a little back and forth movement of the fixator at times, hopefully it is not loosening up and causing discomfort. Continue w/ plan, I recommend attempting a VD radiograph with legs relaxed outwards next time. Dr Gardner

Standing and using right foot relatively normally today. Hard to tell how much weight he is putting on it because he is crouching and the feathers are covering most of leg. When handled kicks surgery leg vigorously and uses foot well. Gently pulled both legs down with him on his back and they extend evenly, which is great because that would suggest the surgery leg is not shorter. Did bicycle PT, good ROM and not really resistant. When extended the leg may curve slightly medially . Cleaned pin sites and applied triple AB Opted to place a perch and he stood on it right away! Also ate everything overnight so didn't force feed. Consider moving out in 2-3 days if continues to eat. Dr Gardner

Sitting toward the back of the kennel when I checked on early in the morning and was towards the front when we went to treat. The boot's additional piece is hanging off. The bird did have the boot under himself and flat on the ground. The bird moved around while attempting to handle, though his other foot got caught in the loose bits of the boot. Opted to change out boot under 2.5% ISO via mask. The pin sites look dry and clean. The cast material is coming off at the distal end a little. Cut the part hanging off and covered in vet wrap. Boot showed signs of wear. Woke up well and upon placing bird back in he stook on the L foot. He can use the R but is resting it upright most of the time now.

Evaluated briefly. Bird stands but is a bit lethargic and does not put full weight on the right leg. Evaluated the ex fix - it is difficult to see through all the fluff, but all pin sites look good and there is minimal bruising. Subjectively it looks like the femur curves medially slightly, and on the radiograph there is a bit of misalignment. It is possible that the limb is curving slightly and the bird needs to adapt to it. Also, it is possible that the limb will be functionally shorter - if so that might explain why the bird is having trouble placing the foot. For now, recommend changing Tramadol to Buprenorphine for 2-3 days to see if that helps. Dr Gardner

Quiet, standing on the left leg with the right foot out. 7g lo. Opted to not weigh since he now has a boot and fixation on. Keel is a 4 but the muscle has started to atrophy a little. Re wrapped the boot since the tape was coming off. There are signs that the boot is being used with depression marks on the boot were the toes are. Opted to reinforce with a lot of tape to see if he will stand. Didn't replace the donut. Went back into kennel and put R foot out again.

Procedure - Fracture repair - femur | Performed by - Dr. Ritzman | Surgery time - 90 minutes | Right Femoral Fracture Repair Surgery:
Patiet placed under general anesthesia, intubated and placed
in sternal recumbency with leaning left to allow access to right
pelvic limb.
The fracture was a closed fracture. Moderate fresh bruising present
with dermis and muscle tissue over the fracture site. There was
moderate, palpable subcutaneous emphysema present as well as
moderate edema of the soft tissues overlying the fracture site.
A longitudinal dermal incision was made over lateral aspect of
the right femur. The muscle bellies were gently and bluntly dissected
apart to expose the proximal and distal femur fracture fragements.
There were two pieces of the femur that had fragmented away from
the main bone but had muscle attachment. One of these butterfly
fragments was able to be preserved and placed back next to fracture
site with the muscle attachment at time of closure.
A 0.062 IM pin was inserted at fracture site and advanced up
throug proximal fragment and advanced out through hip. The distal
end of the IM pin was then advanced down to fracture site and
into the distal fragment with the femur realigned. There was
a large section of the medial aspect of the proximal fracture
fragment absent from the trauma and there was long angle of this
fracture. The femur realigned well with the IM pin placement.
Two 0.045 cross pins were placed from lateral to medial both
in proximal and distal fracture fragments. The muscle fascia
and subcuticular tissues were closed with 4-0 PDS II suture in
simple continuous pattern. The dermis closed with 4-0 PDSII suture
in simple interrupted pattern. Small amount of tissue adhesive
applied to incision as well.
Radiographs were taken of the repair prior to bending and placing
external bar. The radiographs show good alignment of the femur.
The cross pin proximally did not engage distal cortex and was
adjusted (repeat radiograph was not taken after that adjustment).
The distal cross pin was pulled out slightly based on imaging
findings and does engage both medial and lateral cortices.
The pins were bent into external fixator and the cast material
was used to incorporate all of the pins into one external fixator
bar.
Patient recovery was uneventful and smooth.

BADO hit by finder's daughter's car last night. Upon admission, bird was crouched in box, kicked when picked up, quiet in hand. NSF on PE aside from minor bruising and femur fx. Administered torb. Opted to take intake rads in Dirty Box under 3% iso. Rads confirmed an oblique, fresh femur fx, midshaft/proximal, appears closed. Recovery uneventful. SC contacted TR for potential surgery this week. Waiting for response. For now, set up in KR2 kennel with towel and donut. Bird sat up on hocks, eyes closed.
